I confirm that my pet has not attended doggy daycare, boarding or playdates for 2+ weeks prior to boarding my pet and has not gone to any pet stores or other location with high dog traffic.

I confirm that my dog is in good health and does not have diarrhea, coughing, sneezing, vomitting or other signs of illness, and that if my pet does begin to show any of these signs prior to boarding, I will alert Samantha St.Clair of my pet’s illness and have a backup plan for boarding in the event of an illness beginning prior to boarding.
