I confirm that my pet has not attended doggy daycare, boarding, or playdates for 2+ weeks before boarding, and has not visited any pet stores or other locations with high dog traffic.

I confirm that my dog is in good health and does not have diarrhea, coughing, sneezing, vomitting or other signs of illness, and that if my pet does begin to show any of these signs prior to boarding, I will alert Samantha St.Clair of my pet’s illness and have a backup plan for boarding in the event of an illness beginning prior to boarding.
